Abstract

A blank form comprising: a paper substrate having a first area and a second area;cohesive located in the first area and in the second area so positioned that when the form is folded cohesive located in the first area contacts cohesive located in the second area; and a barrier layer interposed between the paper substrate and the cohesive in the first area,so that the cohesive in the first area is not in direct contact with the surface of the paper, and the cohesive in the second area is in direct contact with the surface of the paper.
