Abstract

Systems and methods are disclosed for dynamic radiotherapy treatment planning in a continuous space of computation. Example operations for generating treatment plan data for a radiotherapy treatment include: obtaining data for a radiotherapy treatment of a human subject; generating a set of radiation controls from the data for the radiotherapy treatment, with at least one of the radiation controls being based on a mapping from a continuous (e.g., infinite dimensional) computational space; converting the generated set of radiation controls to a set of treatment delivery parameters, the set of treatment delivery parameters corresponding to capabilities of a radiotherapy treatment machine; and producing treatment plan data for the radiotherapy treatment based on the set of treatment delivery parameters.

Abstract

The present invention relates to the field of radiation therapy and methods, software and systems for adaptive radiotherapy. There is provided a method for adaptive radiotherapy treatment of a patient comprising receiving a sequence of measurement data of the treatment, the measurements being captured at different time points, mapping measurement data to a representation of a treatment geometry at the time points, using the representation in a dynamical model describing how variables in the representation evolve over time, based on the dynamical model, estimating positions over time of the treatment geometry or selected parts of the treatment geometry, determining a treatment action for the patient at a defined time point using the estimation from the dynamical model and executing the treatment action at the defined time point.

Abstract

Image reconstruction can include using a statistical or machine learning, MAP estimator, or other reconstruction technique to produce a reconstructed image from acquired imaging data. A Conditional Generative Adversarial Network (CGAN) technique can be used to train a Generator, using a Discriminator, to generate posterior distribution sampled images that can be displayed or further processed such as to help provide uncertainty information about a mean reconstruction image. Such uncertainty information can be useful to help understand or even visually modify the mean reconstruction image. Similar techniques can be used in a segmentation use-case, instead of a reconstruction use case. The uncertainty information can also be useful for other post-processing techniques.

The method further includes determining whether a condition for terminating the iteration process is met and terminating the iteration process based on a result of the determination that the condition for terminating the iteration process is met.

Much of the image processing that is applied to medical images is a form of "inverse problem". This is a class of mathematical problems in which a "forward" model by which a signal is converted into dataset is known, to at least some degree, but where the aim is to reconstruct the signal given the resulting dataset. Thus, an inverse problem is essentially seeking to discover x given knowledge of A(x)+ noise by finding an appropriate reconstruction operator A† such that A†(A(x)+ noise) ≈ x, thereby enabling us to obtain x (or a close approximation) given knowledge of an output dataset consisting of A(x)+noise. Generally, several such processes (or their equivalents) are applied to the image dataset. If the first process (for example, noise reduction) is expressed via a first reconstruction operator A1† characterised by a parameter set Θ1 and the second process (for example, segmentation) is expressed via a second reconstruction operator A2† characterised by a parameter set Θ2, then the result of the two steps applied consecutively is A2†(A1†(y)). If we then allow a machine learning process to optimise P+, then the steps previously carried out separately can be combined into a single optimisation. This yields advantages in terms of computational load and in the accuracy of the end result.

Abstract

The disclosed systems and methods for Image-Guided Radiation Therapy (IGRT), utilises an iterative approach which adjusts a treatment plan based on inter- or intra-fraction images to improve the accuracy of the radiation delivered during the overall treatment. The prescribed dose of radiotherapeutic radiation is mapped onto the patient's anatomy using an image acquired of the region, which is to be the target for radiotherapeutic radiation. Following beam-angle-optimisation, fluence optimisation and segmentation, the efficiency of delivery of each segment is determined using an objective function, and the segments ranked according to their efficiency. The plan proceeds with the choice of the most efficient segment (or segments) to be delivered first. When this radiation has been delivered, having been tracked to establish its distribution, this delivered distribution can be subtracted from the original prescribed dose and the process repeated so that the delivered radiation gradually converges on the original prescribed dose.

Abstract

A fluorescing marker is used in order to mark (for example) a leaf of a multi-leaf collimator and/or the reference points within the field of view. The markers are illuminated with light tuned to cause the markers to fluoresce at a wavelength different to that of the illuminating light. The fluorescence is then detected by a camera. This method allows the image to be captured by the camera with increased contrast. Accordingly, the present invention provides a multi-leaf collimator for a radiotherapeutic apparatus, comprising at least one leaf having a fluorescent marker. The fluorescent marker will usually emit light of a wavelength longer than the incident light, allowing suitable filters to be provided in order to distinguish the light emitted by the markers. A suitable material for use in the fluorescent markers is ruby. The present invention also provides a radiotherapeutic apparatus comprising a multi-leaf collimator as defined above, and a camera arranged to view the fluorescent markers. A source of illumination for the fluorescent markers is ideally monochromatic, or nearly so. The camera can have a filter arranged to substantially prevent light of the wavelength emitted by the source of illumination from entering the camera, thereby improving the contrast of the image. The radiotherapeutic apparatus can also comprise a source of illumination that is optically co-located with a radiation source, to allow the radiation field that will be emitted to be checked visually by an operator. The co-located source is preferably substantially monochromatic, emitting substantially no light at the wavelength of the fluorescent markers. A filter can then be placed over an output of the radiotherapeutic apparatus, for blocking light of the wavelength of the fluorescent markers and thereby enhancing the contrast of the image that is taken of the fluorescent markers.
